This is the form that all patients must fill out prior to receiving services. Printing this form and filling it out before your visit will help speed up the check-in process. There will still be a section on the actual exam form that requires patient input when you arrive for your appointment, but the majority of writing will be done on this form. Please read over the financial policy portion of the form and sign the bottom, indicating that you agree to the terms.

As of Febuary 8th we will have new hours on Monday and Wednesdays. We will be open from 9:30-7pm on Wednesdays. Monday will go back to normal hours of 10am-6pm. We are open certain Saturdays by appointment only
